FAA MEDICAL EXAMINATIONS

Dr. Angel Soto is certified for FAA Medical Certificate Class I, II and III.

Step by step for you application:

  1. Fill the application: Go to https://medxpress.faa.fgov and follow instructions.

  2. After filling the application make note of the Confirmation Number (8 digits)

  3. Call Dr. Angel Soto’s Office, Wellness Alliance at 787-708-6777 or send an email to info

    @prevencionpr.com for an appointment and bring us a Confirmation Number.

  4. The day of your appointment you will pass (depending on the Class Certificate

    application)

    1. Visual Test

    2. Hearing Test (if applicable)

    3. EKG (if applicable)

    4. Urine test for proteins and blood

    5. Physical Exam

  5. After meeting FAA regulations, a Medical Certificate will be printed and signed by Airmen and AME.

 

Costs:

Class I with EKG - $120.00

Class I w/o EKG - $100.00

Class II and III - $80.00

Application Process for Medical Certification


First Class Medical Certificate: A first class medical certificate is valid for the remainder of the

month of issue; plus

  • 6 calendar months for operations requiring a first-class medical certificate if the airman is age 40 or over on or before the date of the examination, or

  •  12-calendar months for operations requiring a first-class medical certificate if the airman has not reached age 40 on or before the date of examination, or

  •  12 calendar months for operations requiring a second-class medical certificate, or

  •  24 calendar months for operations requiring a third-class medical certificate if the airman is age 40 or over on or before the date of the examination, or

  • 60 calendar months for operations requiring a third-class medical certificate if the airman has not reached age 40 on or before the date of examination. *

Second Class Medical Certificate: A second class medical certificate is valid for the remainder of the month of issue; plus

 

  • 12 calendar months for operations requiring a second-class medical certificate, or

  • 24 calendar months for operations requiring a third-class medical certificate, if the airman is age 40 or over on or before the date of the examination, or

  • 60 calendar months for operations requiring a third-class medical certificate if the airman has not reached age 40 on or before the date of examination. *

  • Third Class Medical Certificate: A third-class medical certificate is valid for the remainder of the month of issue; plus

  • 24 calendar months for operations requiring a third-class medical certificate, if the airman is age 40 or over on or before the date of the examination, or
